More about Jessica
What is one thing you would want patients to know about you?
I believe in compassionate, individualized care and meeting people where they are at.
How do you go about making your treatment plans for FORM patients?
I like to collaborate closely with patients to assess their level of readiness for change. I then like to find out what aspects of their health they are most motivated to learn about, alter and improve. We then work together to build small, realistic goals that are achieved over time.
Why do you love working with FORM patients?
I love connecting with patients all across the country via FORM's virtual platform; it is so rewarding for me to educate and empower patients to reach their goals!
