Claire Pauley, PA-C at FORM™ is passionate about providing individualized, evidence-based care to patients struggling with their weight. She brings a positive energy and collaborative style to each visit to ensure that patients feel supported and empowered throughout their lifelong health journey. Claire earned her Master of Medical Science degree from Midwestern University in 2017 and is a nationally board certified Physician Assistant. She holds a Certificate of Advanced Education in Obesity Medicine from the Obesity Medicine Association. Claire developed a specific interest in treating the root-cause of diseases early in her career and has specialized in chronic disease and weight management since 2018. Claire lives in Tucson, Arizona with her husband and their many backyard chickens. Outside of work, she can be found running, hiking, experimenting in the kitchen, trying new restaurants or playing a mean game of Scrabble!

We are a team! I am here to support patients through all of their successes and encourage them through the inevitable challenges that occur along their health journey!
How do you go about making your treatment plans for FORM patients?
It is important to start by getting to know each patient’s unique history and goals. We discuss their weight history, medical diagnoses, current lifestyle habits and any barriers to health that may be present. Together, we use this information to develop an individualized treatment plan that best suits each patient's specific preferences and long-term health goals.
Why do you love working with FORM patients?
When patients come to us, they have often been struggling with their weight for years and may feel discouraged. I love that we are able to provide patients with hope and long-term solutions for their weight and health. I love that we can take the time to truly get to know our patients and celebrate with them as they achieve their health and wellness goals, both big and small!
