She studied Nutrition and Public Health at New York University and completed her clinical training at Beth Israel Deaconess Medical Center, a Harvard Medical School Hospital in Boston. Julia worked as a bariatric dietitian at NewStart Bariatrics in St. Louis, MO and went on to become the Bariatric Program Coordinator where she fully developed and launched the center’s first non-surgical, medical weight loss program. She has her Certificate of Training in Obesity Interventions from the CDR and in 2022 received the Weight Management Excellence in Emerging Practice Award.

Dr. Jean Baptiste completed her medical training at Nova Southeastern University. She is board certified in Family Medicine and a diplomate of the American Board of Obesity Medicine. She is passionate about health, wellness and promoting lifelong changes and preventing obesity associated diseases; and helping others discover their passion and happiness throughout their wellness journey for long-term success.

Dr. Marshall-Bergman completed her clinical training at The University of Illinois at Chicago. She is board certified in Internal Medicine and is a diplomat of the American Board of Obesity Medicine. Dr. Marshall-Bergman provides care that stems from a desire to form meaningful relationships with and improve the lives and health of her patients. She believes that successful weight management involves focusing on more than simply weight and practices collaboratively to identify and implement small, sustainable changes that help her patients achieve long term success. She has a particular interest in behavioral modification and mindfulness in relation to obesity treatment and wellness.

Julia is a hard-working, Registered Dietitian at FORM™ with lots of experience in weight management and nutrition counseling. She is passionate about guiding and supporting her patients during their weight loss journey. Julia attended the University of Georgia where she received her Bachelor of Family and Consumer Sciences in Dietetics. Julia then continued her education and received her Master of Science in Health Sciences at Georgia State University. While Julia attended Georgia State University, she also completed her Dietetic Internship. Julia is a Board Certified specialist in Obesity and Weight Management, and works with her patients to create a personalized program that is the best fit for them. 

Emily is an experienced Registered Dietitian with 13 years of nutrition counseling experience. At FORM™, Emily is dedicated to providing obesity treatment access to her patients and working with them to personalize a lifestyle weight loss plan that they can realistically follow. Emily graduated from Keene State College in New Hampshire with a Bachelor of Science in Health Science with a dual major in Nutrition and Health Promotion and Fitness. She then continued her learning by completing her Dietetic Internship at Saint Joseph College in West Hartford, Connecticut. 

Dr. Frederiksen completed her clinical training at the University of Rochester and the University of Colorado. She has a Masters degree from the Harvard Chan School of Public Health.Before joining Form Health, she practiced Internal Medicine for over twenty years and started  two successful medical weight management programs. She is a diplomate of the American Board of Obesity Medicine. She believes strongly in providing patients with an individualized  approach to weight loss, along with a great patient experience.

Casey is a Registered Dietitian at FORM™  that specializes in weight management. Her goal is to create a realistic plan that is individualized based on her patients medical history and needs. She is passionate about getting to know her patients and is driven by their motivation. Casey got her Bachelor of Science in Human Ecology with a major in Nutrition at the University of Tennessee. She then received her Masters of Science in Clinical Nutrition at the University of Kentucky. After school, she completed her Dietetic Internship at Brigham and Women’s Hospital in Boston, MA.

Dr. Lowden is an endocrinologist who completed her clinical training at Northwestern University in Chicago. She is dual-board certified in Endocrinology, Diabetes, and Metabolism as well as Internal Medicine. She is also a diplomate of the American Board of Obesity Medicine. Dr. Lowden supports individualized care while considering all facets of a patient's lifestyle and health. She is passionate about helping patients achieve their health goals and improve metabolic diseases.

Dr. Smith completed her clinical training at Northwestern University in Chicago, Illinois. She is board certified in Internal Medicine, Endocrinology, Diabetes, & Metabolism, and a diplomate of the American Board of Obesity Medicine. Dr. Smith helps patients with a variety of underlying medical conditions improve their health through weight loss and individualized, lifestyle modifications that assist in achieving sustainable results.
