Since its founding in 2019, Form Health has provided care to patients across the United States via its unique telehealth care model, which includes a dedicated clinical Care Team consisting of an American Board of Obesity Medicine (ABOM) certified doctor and a Registered Dietitian who is a Certified Specialist in Obesity and Weight Management (CSOWM).
