This year’s list, featured on Inc.com, is the result of comprehensive measurement and evaluation of American companies that have excelled in creating exceptional workplaces and company cultures–whether in-person or remote. The award process involved a detailed employee survey conducted by Quantum Workplace, covering critical elements such as management effectiveness, perks, professional development, and overall company culture. Since its founding in 2019, Form Health has provided care to patients across the United States via its unique telehealth care model, which includes a dedicated clinical Care Team consisting of an American Board of Obesity Medicine (ABOM) certified doctor and a Registered Dietitian who is a Certified Specialist in Obesity and Weight Management (CSOWM). The Care Team develops an individualized weight loss plan for each patient that addresses behavioral health, nutrition, and physical activity and includes a prescription for FDA-approved weight loss medication, if appropriate. This approach results in patients hitting their weight loss goals, improving their overall health, and creating sustainable change.
